醫(yī)療體系應(yīng)該全面支持母乳喂養(yǎng)
“整個醫(yī)療體系應(yīng)該接受跨科室培訓(xùn),通過系統(tǒng)性醫(yī)療課程全面了解母乳喂養(yǎng)是一種疾病預(yù)防措施,能夠預(yù)防生命各個階段的多種疾病。”Paula Meier 教授是拉什大學(xué)醫(yī)學(xué)中心新生兒重癥監(jiān)護(hù)病房 (NICU) 泌乳支持主任,也是全球 NICU 泌乳支持領(lǐng)域最受歡迎的專家。雖然全球眾多醫(yī)學(xué)倡議已認(rèn)識到,大劑量親母母乳對于早產(chǎn)兒十分重要,有助于減少致命的壞死性小腸結(jié)腸炎、遲發(fā)性敗血癥、支氣管肺發(fā)育不良(嚴(yán)重的呼吸系統(tǒng)疾?。?、再次入院以及后續(xù)的神經(jīng)發(fā)育問題。然而,全球并未實(shí)施標(biāo)準(zhǔn)化基于循證學(xué)的方案,以支持這些全球倡議。Meier 教授從 1996 年拉什大學(xué)醫(yī)學(xué)中心啟動的“親母母乳支持項(xiàng)目”中汲取經(jīng)驗(yàn)教訓(xùn),提出了在全球 NICU 實(shí)施新型標(biāo)準(zhǔn)化母乳喂養(yǎng)支持最佳臨床路徑,提出醫(yī)院里干擾早產(chǎn)兒母乳喂養(yǎng)的七大障礙,以及如何通過低成本措施和同伴咨詢系統(tǒng)來克服這些障礙。
